| 2008 Permission | This permission slip must be completed and on file for every Troop 91 outing. |
| Class 1 and 2 Medical Form | The Class 1 Medical form must be completed with a parental signature every year. The Class 2 medical form requires a doctor's signature and is good for two years. The Class 1 and 2 Medical forms must be on file for Summer Camp or any official BSA outing, such as an OA Ordeal. |
| Class 3 Medical Form | The Class 3 Medical Form requires a physical exam and is good for one year. Scouts who engage in high-adventure activties (such as "Mountain Man" at Summer Camp) must have a Class 3 Medical form on file.
Adults over 40 must have a completed Class 3 Medical Form to participate in any official BSA activity. |
